ABSTRACT:
The integration of herbal medicine with conventional (allopathic) pharmacotherapy has attracted growing scientific interest over the past decade, driven by the promise of enhanced efficacy, reduced drug dosage, and mitigation of adverse effects through synergistic interactions. This review synthesizes clinical evidence (2015–2025) on herb drug combinations across oncology, endocrinology, cardiovascular medicine, hepatology, and musculoskeletal disorders. A narrative synthesis of randomized controlled trials, systematic reviews, and meta analysis indexed in Pub Med, Science Direct, and Cochrane Library was performed, focusing on curcumin chemotherapy combinations, berberine metformin co-therapy, ginger antiemetic regimens, silymarin antitubercular co-administration, garlic antihypertensive combinations, green tea catechin anticancer drug pairings, and Boswellia/curcumin NSAID combinations. Consistent evidence indicates that several herb drug pairs produce clinically meaningful synergism. However, methodological heterogeneity, small sample sizes, and variable herbal standardization limit definitive conclusions. Herb drug synergism is mechanistically plausible and clinically supported for select combinations, but rigorous, adequately powered, and well standardized trials are needed before broad clinical translation.
